OVERVIEW

Join this interactive virtual training for CHWs to gain a strong foundation in diabetes care and education. Tailored to the Competency Framework from The Association of Diabetes Care & Education Specialists, this course will provide a dynamic overview of diabetes along with the role of CHWs can play in supporting those living with diabetes. A Q&A will be incorporated into this session from an experienced diabetes educator. 

OBJECTIVES

  • Provide overview of types of diabetes and general pathophysiology.
  • Review common complications of diabetes and ways to prevent.
  • Overview of Diabetes Self-Care Behaviors Framework
  • Overview of what DSMES is
  • Role of the CHW in supporting patients with diabetes.

Chronic Disease Management Coordinator, NH Healthy Lives

17 years of clinical experience working directly with patients as a dietitian and diabetes educator prior to joining NH DHHS.
