The HAP Foundation, in partnership with Education in Palliative and End of Life Care (EPEC), is offering a 12-month clinical training program focused on adult palliative care in home- and community-based settings, specifically designed for rural Illinois providers.

This clinically focused program includes monthly virtual training sessions via Zoom, along with ongoing networking and collaborative learning opportunities throughout the year. The cohort year will begin and conclude with full-day, in-person educational events.

Curriculum topics will include advance care planning, having difficult conversations, whole-patient assessment, pain and symptom management, goals of care,  effective use of the interdisciplinary team, and cultural competence in home-based care. Virtual sessions will provide opportunities for participants to present cases and receive peer support and clinical guidance from a team of expert faculty.

The program will launch with a full-day, in-person conference on October 2, 2026, in central Illinois. Additional details will be shared soon.

Continuing Education credits will be offered for Advanced Practice Nurses, Registered Nurses, and Licensed Social Workers.
